- PPO, a preferred provider organization plan that gives you the freedom to choose any licensed physician without a referral, in- or out-of-network. Select from the Plus, Preferred, Prime, Value and Health Savings Account (HSA) plans.
- Traditional Indemnity, a plan that gives you the freedom to choose any licensed physician without a referral. Select from the Choice and HSA Choice plans and customize your policy with different levels of benefits, deductibles and coinsurance.
- HSA, the HSA Choice and HSA PPO plans may be complemented with a health savings account plan. Deposits made in the HSA account are tax-deductible, and the accumulated savings remain in the account even if your employment status changes.
- Frontline™, a selection of preferred provider organization (PPO) plans that offer members the freedom to visit any licensed physician of their choice. A $20 copayment is required for each hospital visit, and the first 6 visits to Preferred Providers are completely covered by LifeWise.
- Passport™, a selection of preferred provider organization plans that offers members the freedom to visit any licensed physician of their choice. Pay a set copayment, and the deductible is waived when members visit a Preferred Provider.
- Medicare Supplement Coverage, a selection of supplemental benefit plans that allow Oregon seniors to enhance their Medicare health coverage. Select from plans A, C, F and I.
