Select from over 25,000 licensed physicians or 173 hospitals in our extended network, or someone of your choice. No referrals necessary.

  • Copay Options, similar to Distinctions, is a plan that allows the policy holder the flexibility to visit any primary care physician in our extended network. Depending on the superiority of service and prices, providers are assigned to different tiers. Those who visit Level 1 primary care physicians will save the most money.
  • Primary Clinic, an established health maintenance organization (HMO) plan that allows the consumer to select a primary care physician (PCP) from the network. All other doctors and specialists visited must be referred by the PCP.
